In order to refer a patient to our practice for a periodontal evaluation, please fill out the Referral Form and fax or e-mail it to us. If you have a recent digital radiograph of the area of concern, please e-mail it as well. After examining your patient, Dr. Ghiabi will send you a written report, detailing the treatment plan. Once the patient is treated and the area monitored for healing, the patient will be sent back to your office. You will also receive a final report of the treatment rendered and its outcome. At any time, please do not hesitate to contact us should you have any questions regarding this process.
